The vesicles or pustules eventually rupture to leave the characteristic honey-colored (yellow-brown) crust. Hypercalcemia usually, but not always, resolves if therapy with lithium is discontinued. The prevalence of lymphomas tends to be much higher than their incidence, given their natural history and availability of effective therapies. Long-term use of bisphosphonate therapy has been associated with osteonecrosis of the jaw. A careful physical examination may disclose skin or mucosal lesions in about 50% of cases. Although generally self limited, Salmonella infection may cause sepsis and localized infections, such as septic arthritis and infection of endovascular prosthetic devices, such as grafts. Uncommon diseases such as anthrax, rabies, plague, and Japanese encephalitis are also preventable with immunizations. This must be explained to skeptics, emphasizing that the gains of vaccination clearly outweigh its potential risks, and that respiratory illnesses caused by other infec tious organisms are not prevented by the vaccine. Although the serum ferritin level is useful when there is no accompanying inflammation, it is influenced by acute phase responses. An international panel of experts has created a robust although unwieldy scoring system that delineates most patients with this disorder (Table 1). The most common adverse effect with voriconazole is a transient and reversible visual disturbance in 30% of cases. Other causes include severe left ventricular dysfunction resulting in a low ejection fraction, paradoxical embolus from the venous system due to a shunt through a septal defect such as an aneurysm or patent foramen ovale, or vessel-to-vessel atheroembolism due to atherosclerotic disease in the vertebral arteries, carotid arteries, and aortic arch. The outpatient management of these patients in my institution usually involves frequent visits to receive platelet transfusions. Spinal cord compression is relatively common, and a high index of suspicion must be maintained for patients presenting with back pain, even in the absence of neurologic findings. Antihypertensive agents may be resumed if the patient continues to be hypertensive. Certain patient populations, however, may be considered for adjuvant treatment, including patients with inadequately sampled nodes, T4 lesions, perforation, or poorly differentiated histology. In addition, in patients with idiopathic polyneuropathy, judicious reassessment of their laboratory investigations should be performed periodically, particularly if symptoms and signs progress. Other features include scaly, telangiectatic plaques with atrophy and hypopigmentation (poikiloderma) on the face, neck, trunk, and extremities; malar erythema; and nail abnormalities (periungual telangiectases and cuticular hypertrophy). These criteria continue to be revised by our urology colleagues, tending to become more stringent with time. Thrombosis in larger vessels can lead to myocardial infarction, deep venous thrombosis, transient ischemic attacks, and stroke. Comparisons of the earliest and most recent autopsies and barium enema studies have indicated that the worldwide prevalence is increasing over time (Box 2). Additional displacement of blood volume into the systemic circulation during contractions makes labor particularly hazardous. The 1-year prevalence of adjustment insomnia is estimated to be about 15% to 20% and is more common in women and older adults. Patients with idiopathic chronic back pain, whiplash, and fibromyalgia, conditions often believed to be exaggerated or psychogenic, show evidence of central sensitization, again suggesting that their pain is genuine but not due to peripheral structural pathology. Diagnosis of vaginal trichomoniasis is by microscopic examination of a wet mount, which is 60% to 70% sensitive. The specific regimen may be altered if the source patient has known resistant virus. Cases of transmission from birds to humans with severe disease have led to international concern about a possible avian influenza pandemic. Typically, tests using molecular methods are more expensive than conventional viral cultures. Box 5 lists the accepted indications for revascularization in patients presenting with intermittent claudication. Dizziness that is out of proportion to the observed nystagmus may also be observed. Specific neuropsychological or neurophysiologic testing may be required to secure the diagnosis in these cases. Such abnormal inclusions include intraerythrocytic parasites such as Babesia species. Both patient and physician must realize that several months of therapy are needed to replenish body iron stores. None of these primary headaches is associated with demonstrable organic disease or structural neurologic abnormality. Combination of pharmacologic and endoscopy therapy is employed for secondary prevention of variceal hemorrhage. Pneumatic Dilation Dilation with bougies and small balloons produce brief results. Recommended therapy for episodic treatment is oral therapy for 5 to 10 days with acyclovir, 400 mg three times daily or 200 mg five times daily, famciclovir, 500 mg twice daily, or valacyclovir, 1 g twice daily.
