When O2 delivery is so severely impaired such that oxidative phosphorylation cannot be sustained, the state is termed dysoxia. The boundaries of the space are separated by the styloid process and its associated fascial attachments into the "prestyloid" and "poststyloid" compartments. The metastatic process begins with an in situ cancer surrounded by an intact basement membrane. Diagnosis can be achieved through serum analysis for anticoccidioidal antibody, spherule identification in tissue, or by isolating the fungus in cultures from sputum, other body fluid, or tissue. In high risk patients who have received 5 years of adjuvant tamoxifen extended adjuvant therapy with at least 3 years of an aromatase inhibitor has been shown to provide a significant benefit in terms of disease outcome.
