Failure to place an adequately sized roll may lead to excessive stretch of the brachial plexus, with the greatest effects on the C5 and C6 nerve roots. Unfortunately, there are significant barriers to using most large databases for this purpose, especially administrative databases. This general topic and many of the specific research methods used are directly applicable to neurosurgeon-scientists, particularly those engaged in the surgical treatment of patients with epilepsy. The progress in its surgical treatment is not comparable, however, to the sophisticated development of treatment concepts and surgical techniques in other subspecialities of neurosurgery, such as functional, spinal, or vascular neurosurgery. Usually based on interview or questionnaire, such studies can generally be accomplished relatively inexpensively and quickly in comparison to the cohort studies that they mimic. Although it does not take into account surgical risk and is not primarily designed for prediction of outcome, it has been found to correlate with perioperative morbidity and mortality. In patients with pituitary adenomas, the visual field and endocrine function need careful evaluation. In one series of 142 patients with brain abscess,13 there were no significant differences in outcome in those who were treated by excision, craniotomy with drainage, or stereotactic drainage, thus indicating that therapy must be individualized for each patient. Vestibular and cochlear signs and symptoms are common and occur in approximately 50% of patients. There are several probable reasons for this discrepancy, including a higher incidence of medial temporal lobe epilepsy that is intractable, better seizure-free rates with medial temporal lobe resection than with extratemporal resection, and frequently, more diffuse or obscure extratemporal seizure foci necessitating a more tailored approach for extratemporal resection and requiring additional evaluation for determination of the exact seizure location, often with surgically implanted electrodes. Inflammation at the site of infection may facilitate entry of drugs across these barriers and into the brain, but not all postoperative infections are accompanied by marked inflammation, and concomitant treatment with corticosteroids may further impair drug entry. A poor initial trajectory may not be remediable by attempted endoscopic catheter placement. In evaluating patients according to these three overarching criteria, it is important to note that intractable epilepsy is a progressive disorder that is medically, physically, and socially disabling. Several case studies and series have demonstrated efficacy in patients in whom medical treatment has failed. Neurogenic pulmonary edema can also occur because of the intense sympathetic discharge. Inhibition of these channels is thus a double-edged sword that promotes local increases in excitation important to such processes as learning and memory while also potentially rendering areas of the brain proepileptic. Although dominant temporal lobe surgery predisposes the patient to a decline in verbal memory and short-term memory functioning, the risks associated with nondominant temporal lobe surgery are less predictable. Care is taken to avoid damaging the electrode, the generator is removed, and the electrode or electrodes are disconnected from the generator after the set screws have been loosened. We are not aware of any clinical study demonstrating an advantage of smaller steps, although changes as small as 10 mm H2O can result in clinical responses. A typical example of this is the requirement for central venous Doppler monitoring for air emboli in the case of sitting craniotomies.
