Finally, because of the potential for renal dysfunction and respiratory insufficiency in pregnant women with acute pyelonephritis, careful monitoring of renal function, urinary output, and respiratory status, including pulse oximetry, is necessary. The intrahepatic portion of the umbilical vein may be the preferred target for vascular access when transfusing a twin gestation if the corresponding placental cord insertions are difficult to identify. If a susceptible woman subsequently is exposed to rubella, serologic tests should be obtained to determine whether acute infection has occurred. In some studies, small amounts of hemorrhage increased plasma renin activity,57,58 but other studies have shown little effect. Whenever possible, the operation should be performed before pregnancy; otherwise, the maternal mortality rate is approximately 3%. Immune hemolysis is related to alloantibodies, drug-induced antibodies, and autoantibodies. The heart should occupy no more than approximately one third of the cross-sectional area of the thorax. The rationale was that minimally invasive access to the amniotic cavity would reduce the frequency of preterm labor and diminish maternal morbidity. This approach to gating, first described in 1996,147,148 has become widely commercialized as spatiotemporal image correlation. Broad-based preventive strategies such as progesterone supplementation can be considered for those at risk as a result of less specific risk factors, such as a history of spontaneous preterm birth (see Chapter 40). When serum biochemical markers measured at 10 weeks were also added, the modeled detection rate increased to 92% with a 5% falsepositive rate, or 84% with a 1% false-positive rate. Using the Missouri maternally linked data containing births from 1978 to 1997, the risk for stillbirth recurrence among relatively low-risk women was studied. The presence of serum intrinsic factor antibodies is specific for pernicious anemia. Adulthood adiposity, insulin resistance, and hypertension have also been induced by maternal nutritional intervention, both undernutrition and overnutrition. In practice, the distinction between direct and indirect is more useful for understanding causes than for clinical care. Adhesive bands result from a broad fusion between intact amniotic membranes and disrupted fetal parts. The estimated risk is about 1% to 2%, with higher doses thought to be associated with higher risk. This should demonstrate that the individual has fully understood the multiple options that ensue from testing. The cleft in the mitral valve typically does not generate signs or symptoms unless it is associated with more severe degrees of mitral regurgitation. At the arterial end of the exchanger, the maternal bloodstream enters with a higher Po2 than fetal blood (72 versus 19 mm Hg), thus establishing a Po2 gradient that drives O2 molecules across the membrane into fetal blood. In this situation, a diagnosis of placental abruption can be established on the basis of fresh macroscopic or histologic examination of the placenta. For women without a previous uterine incision, cesarean delivery should be limited to maternal indications. The environmental exposure was bacterial vaginosis diagnosed by symptomatic vaginal discharge, a positive whiff test, and clue cells on a wet preparation. When used as a tocolytic, nifedipine is commonly given orally with a 10- to 20-mg initial dose, repeated every 3 to 6 hours until contractions are rare, then followed by long-acting formulations of 30 or 60 mg every 8 to 12 hours for 48 hours while antenatal steroids are being administered.
Syndromes
The authors emphasized the need to prevent unnecessary primary cesarean deliveries and identified induction practice as an ideal place to start. Maeno Y, Kamenir S, Sinclair B, et al: Prenatal features of ductus arteriosus constriction and restrictive foramen ovale in d-transposition of the great arteries, Circulation 99:1209, 1999. Ideally, the improved outcomes are accompanied by appropriate interventions and restraint from inappropriate interventions. Giannakoulopoulos X, Sepulveda W, Kourtis P, et al: Fetal plasma cortisol and beta-endorphin response to intrauterine needling [see comments], Lancet 344:77, 1994. A recent study found that women with dilated cardiomyopathy who became pregnant had an adverse event (heart failure hospitalization, arrhythmia, stroke, myocardial infarction, and cardiac death) rate of 40% during pregnancy, and 70% at 18 months of follow-up. Regardless of whether there is a uterine anomaly, the diagnosis of an interstitial ectopic pregnancy can be difficult. This rationale for choosing magnesium sulfate has been challenged by reviews that emphasized the paucity of data to support any benefit. Pillai M, James D: the development of fetal heart rate patterns during normal pregnancy, Obstet Gynecol 76:812, 1990. Risk factors may also serve as markers to identify candidates for cervical ultrasound screening for short cervix. In this example, a number of small trials show a nonsignificant trend in favor of antibiotic treatment. Variability is marked when the amplitude is greater than 25 beats/min66 (see Classification and Significance of Baseline Variability, later). This approach decreases the rate of persistent or recurrent Chlamydia compared with standard partner referral. This can be accomplished with a simple record of cervical dilation plotted against time on ruled graph paper or by a more comprehensive recording of all intrapartum data related in graphic form to the progress of cervical dilation. Reasons for poor responses include injury to 15 Fetal Lung Development and Surfactant 183 the preterm lung by inflammation or ventilatory injury before surfactant treatment, unrecognized pulmonary hypoplasia, or very immature lung structure. In a nationwide Swedish cohort (N = 354,676), women with prior preterm preeclampsia had more than doubled risks of stillbirth, placental abruption, and preterm births and had an even greater risk of giving birth to a small-for-gestationalage infant despite the absence of preeclampsia. The incision is then closed, leaving the equivalent of a classic cesarean incision on the posterior surface of the uterus. In addition, routine cellulose acetate gel hemoglobin electrophoresis cannot delineate all hemoglobin variants. Persistent headache, visual aberrations, or epigastric or right upper quadrant pain 5. There have been studies on the effects of decreasing uterine blood flow,21 decreasing umbilical blood flow,27 shifting the oxyhemoglobin dissociation curve of either fetal blood28 or maternal blood29 to the right, ventilating the maternal lungs with different gas mixtures,29 decreasing the fetal oxygen demand,30 and reducing the placental exchange surface by 50%. Clearly, this exposes the fetus to radiation, so extensive lead shielding should be used. A theme of lung development is that regulatory elements that were essential for embryonic and canalicular lung development are also critical during terminal lung development with alveolar septation, although with different roles and localizations. This definition has physiologic validity, in that it is the amount of Hb per unit volume of blood that determines the oxygen-carrying capacity of blood. Duodenal atresia produces the classic echolucent "doublebubble," which represents the dilated stomach and proximal duodenum (Videos 24-10 and 24-11). However, although the neural activity of fetal motility and its motor effects may contribute to the development of muscles, joints, and even the fine structure of the central nervous system itself,107 there has been little study, either clinical or animal based, in this area to date. Quantitative measurements of the aortic annulus and arch may be useful,96,117,214 as may Doppler evaluation of the aortic isthmus. Baseline information should be obtained to determine renal function, coagulation status, and liver function. Carotid denervation partially inhibits the response, and combined carotid and aortic denervation abolishes it. It is important, if at all possible, to make the diagnosis before delivery, because intraoperative hemorrhage can be massive, and placenta accreta has been reported to be the most common indication for emergency peripartum hysterectomy. Consistent with the right-sided predominance seen at the levels of the atrium, atrioventricular valve, and ventricle, the pulmonary valve annulus should be equal in size or, more commonly, slightly larger than the aortic valve annulus. It seems more likely that types 0, 3, and 4 represent different pathogenetic processes. For example, let us assume the population tested could be selected by adding clinical data to define a group with a higher prevalence of delivery within 7 days. Basso O, Christensen K, Olsen J: Higher risk of pre-eclampsia after change of partner. Women with primary or secondary syphilis are more likely to transmit infection to their offspring than are women with latent disease. Regardless of the method of delivery, the patient should receive intravenous zidovudine during delivery (2 mg/kg for 1 hour, then 1 mg/kg/hr until delivery). The infection may become chronic or progress to septic arthritis and joint destruction. Families need to consider the possibility of a test showing that one of the twins is normal and the other has an abnormality. Because of the poor prognosis associated with hydrops when an abnormal karyotype is involved, such fetuses are generally not considered candidates for in utero fetal therapy or for active intervention with early delivery and vigorous resuscitation. Cigarette smoking in pregnancy is associated with an increase of cervical antiinflammatory cytokines without a commensurate increase of proinflammatory cytokines. Although the hemorrhage may be self-limited, if a fetus has developed hydrops, many authors have advocated more-aggressive management because of the risk for demise. After completion of the fetal portion of the procedure, the uterus is closed in two layers with resorbable sutures, amniotic fluid volume is restored, intra-amniotic antibiotics are administered, and magnesium sulfate is initiated. Similarly, oligohydramnios, a decrease in fetal movement, and poor fetal tone are 37 Nonimmune Hydrops 577 all ominous signs. Likewise, confounding should always be considered as an explanation of observed results. It usually is related to the severity of the event and is particularly likely to occur if there is fetal demise or massive hemorrhage. Turrentine and colleagues reviewed the outcomes of 172 pregnancies treated with thrombolytic therapy for a variety of indications and reported a maternal mortality rate of 1. After an initial ultrasound assessment, repeated evaluation can be performed every 1 to 2 weeks to determine whether there is reaccumulation of amniotic fluid and to evaluate lung growth. The survey places an emphasis on accounting for perceptions of patient safety issues and on error and event reporting. Cells grown in flasks are harvested and analyzed together, whereas those grown on coverslips are analyzed in situ as individual colonies. The onset of action is 10 to 20 minutes, with peak action 20 minutes after administration and a duration of action of 3 to 8 hours. Fetomaternal Hemorrhage Fetomaternal hemorrhage, the transplacental passage of fetal blood cells to the maternal circulation, has been attributed as the cause of about 4% of stillbirths. Abnormalities of fibrinogen can be inherited (typically autosomal recessive) or acquired and are rare, with a reported prevalence between 1 in 500,000 and 1 in 2,000,000.
However, the biologic mechanisms that allow this commensal microorganism to become a pathogen are not completely understood. Cephaloceles can be occipital, parietal, or frontal; in the Western Hemisphere, 80% are occipital. Prior Successful Outcome after Cerclage Prolongation of pregnancy after cerclage placement does not prove a diagnosis of cervical insufficiency, because many pregnancies with premature cervical effacement have good outcomes in the absence of surgical intervention. Although Chesley352 found no increased risk of subsequent chronic hypertension for women with eclampsia in their first pregnancy, mortality was twofold to fivefold higher over the next 35 years among women with eclampsia in any pregnancy after the first. Preterm birth rates were not improved by antibiotic treatment in either trial, and in fact were slightly higher in women who received antibiotics. Placental development had compensated for the lower uterine venous Po2 of the A sheep by producing a smaller uterine-umbilical venous Po2 difference (14. If the patient presents in preterm labor or if symptomatic hydramnios exists, difficult decisions need to be made about whether to administer tocolytic medications or to allow labor to continue. This yields an actual risk that ranges from 15% to 20% to more than 50% to 60% when maternal race, ethnicity, and the number and gestational age of prior preterm deliveries are considered. In rodents, surfactant maturation is complete in the saccular lung, as alveolarization occurs after birth. The normal baseline variability between contractions signifies normal central oxygenation despite the intermittent hypoxic stress represented by the moderate variable decelerations. The lateral incisional borders havebeenstapled with resorbable material (arrows), providing hemostasis. The disorder is characterized by progressive neurologic debilitation and has an almost uniformly fatal outcome. Severe maternal sensitization does occasionally occur before 28 weeks, and by not performing the antibody screen, the clinician loses the opportunity to detect a potentially salvageable anemic fetus. Its shortcoming is its failure to address the cause of the disease, because the vascular anastomoses remain patent. The hypokinetic collecting system reduces urine flow, and urinary stasis occurs, predisposing to infection. However, it is unknown whether this relative loss of muscle mass (sarcopenia) persists into adulthood, although several studies have demonstrated a positive association between birth weight and muscle mass at different stages of life. In the fetus with complete heart block, the atrial rate remains normal but atrial contractions do not conduct to the ventricles. If the fetal membranes do not rupture before labor, the work to cause membrane rupture at the internal cervical os decreases with advancing cervical dilation because of the lack of anchoring to the supportive decidua and the enhanced ability to stretch with contractions. Because up to 20% of such fetuses have malformations or aneuploidy, a diligent review of fetal anatomy and, in some cases, invasive testing for fetal karyotype are indicated. This is not necessarily so in the antepartum period before the onset of uterine contractions. If not at high risk for anaphylaxis: the drug of choice is cefazolin, 2 g intravenously, then 1 g every 8 hours until delivery. In light of the sequelae that can occur in the mother and newborn infant if chlamydial infection persists. Sterile drapes should be applied in such a way that one observer can, with a sterile speculum, examine the vagina and cervix to determine when the bleeding has ceased. If the left-to-right shunt is large, the circulation is hyperdynamic, with a wide arterial pulse pressure, low arterial diastolic pressure, and hyperactive precordium. Although iron supplementation has not been consistently shown to alter perinatal outcome, the Centers for Disease Control and Prevention strongly recommends screening and treatment of iron deficiency anemia in pregnancy. The embryonic neural tube is formed via neurulation, which involves shaping, folding, and midline fusion of the neural plate, and it is typically complete by the 25th day after conception. Delivery of the fetus should be considered in refractory cases, because improvement has been reported in sporadic cases. Because of differences in albumin binding, placental transfer, and glucocorticoid receptor affinity, substantially higher dosages of cortisol, cortisone, hydrocortisone, prednisone, and prednisolone are required to reach dosage equivalency to betamethasone and dexamethasone in the fetus. Finally, because they have been collected for years, they are an excellent resource for trend analysis. Cook D, Guyatt G: the professional metaanalyst: an evolutionary advantage, J Clin Epidemiol 47:1327, 1994. Paralleling its pivotal role in initiating the hemostatic reaction, thrombin plays a central role in initiating the anticoagulant system. Over a period of several weeks, this phase of the illness gradually resolves, and the patient enters the latent phase of infection. However, because delivery is very unlikely to cause massive hemorrhage by itself and fetal cells persist for up to 2 to 3 weeks after delivery, testing soon after delivery is reasonable. Conscious sedation can be used for the procedure and is best managed in an operating room setting with the assistance of an anesthesiologist. Most of the pregnancy would be spent in bed, the course would be punctuated by episodes of uncompensated congestive heart failure (any of which could prove fatal or require therapeutic abortion), and the risk to the fetus would exceed 50%. Sentilhes and colleagues reported on a cohort of 167 women with accreta in whom conservative management was planned before delivery. Ultrasonography is used to map the location of all gestations precisely in the uterus, and measurements are taken of crown-rump length and nuchal translucency thickness. This typically resolves within 24 hours after the medication is discontinued, without sequelae. Normal results may or may not include mention of copy number changes that were identified and believed to be benign. Most of the fluid moves rapidly into the interstitial spaces and then directly into the pulmonary vasculature, with clearance of less than 20% of the fluid by pulmonary lymphatics. The surfactant given as treatment becomes a metabolic substrate for the preterm lung, which can improve its function. Adverse event reports summarized in the product labeling indicates that of 77 pregnancies exposed to mycophenolate, 25 spontaneously aborted, and 14 resulted in a malformed infant or fetus. In preeclamptic women, oliguria can have a prerenal or renal origin (see Chapters 57 and 71). The actual risk of warfarin embryopathy is difficult to estimate: it has ranged from 4% to 67% in various reports. Conversely, surface rendering would be most helpful in the assessment of soft tissue pathology, such as cleft lip. For example, the critical window for an agent that interferes with closure of the neural tube in the human embryo is approximately 21 to 28 days after conception. In one series of 750 diagnostic or therapeutic cordocentesis procedures, the incidence of fetal bradycardia was 21% with puncture of the umbilical artery but only 3% with umbilical venous puncture. With this screening approach, impending intolerable fetal acidosis can be presumed or, in certain cases, ruled out by the use of ancillary techniques. Also, even in pregnancies with stable disease, the hostile intrauterine environment persists and may result in the premature birth of two compromised infants, with the associated risk of neonatal death and morbidity. Yerushalmy J: Relation of birth weight, gestational age, and the rate of intrauterine growth to perinatal mortality, Clin Obstet Gynecol 13:107, 1970. Historically, this was practiced because of concern about interlocking fetal heads in breechvertex twins. This process will undoubtedly identify many candidate genes of interest for translational research and potential clinical application. As stillbirths resulting from placental causes occurred earlier in gestation (median, 30 weeks) compared with unexplained stillbirths (median, 38 weeks), uterine artery Doppler imaging was a moderate predictor of all-cause stillbirth up to 32 weeks (sensitivity, 58%, with a false-positive rate of 5%). In these cases, sampling the lower sac transcervically and the upper transabdominally minimizes the chance of contamination. Placental and fetal infections probably originate from either ascending or hematogenous spread. The pressure of the baseline uterine tone and that of any uterine contraction is translated into an electrical signal, which is calibrated and displayed directly (as millimeters of mercury [mm Hg]). When such studies are performed, the placenta may be classified as a complete previa if it completely covers the internal os. The vast number of mutations with their varying phenotypic consequences frequently require sophisticated laboratory capabilities and subsequent counseling by individuals explicitly trained in this area. The sample sizes are often large, facilitating the evaluation of even rare outcomes. A Joint Commission Sentinel Event Alert provided an overview of the common approaches to preventing failures and latencies inherent in complex systems and human activities. Deep tendon reflexes and vital signs, including respiratory rate, should be recorded hourly. If the paternal blood typing is negative for the involved red cell antigen, further maternal and fetal monitoring is unwarranted as long as paternity is ensured. The risk for early birth rises with the number of fetuses, suggesting uterine overdistention and fetal signaling as potential pathways to early initiation of labor (Table 40-3). The enrichment process may require up to 24 hours, negating the time-saving advantage of the rapid test. This stability on deflation explains the increased functional residual capacity in the surfactant-sufficient lung. American College of Obstetricians and Gynecologists: Human immunodeficiency virus infections in pregnancy. Srinivasan and colleagues have reported a test for more than 100 mendelian disorders, with multiple mutations tested per allele, in which 35% of individuals were found to be a carrier for at least one mutation, and the rate of carrier couples was approximately 0. On the basis of these observations, Iams and colleagues proposed that a short cervical length may represent a spontaneous preterm parturition phenotype characterized by asymptomatic shortening of the cervix, but not decidual and myometrial activation. Some authors have proposed that pericardial effusions indicate cardiac decompensation and that this is the earliest sign of hydrops in fetuses with cardiac lesions. A randomized trial of high-protein versus lower-protein feed in 1138 formula-fed infants revealed that 2-year-olds who were fed the lower-protein formula as infants had reduced weight-for-length z scores compared with those who had been fed the higherprotein formula, and that z scores in the lower-protein group were similar to those in a breastfed reference group. These changes are more consistently correlated with proteinuria than with hypertension. Jejunoileal atresia produces multiple dilated loops of bowel with increased proximal peristalsis. Typically, a blood sample is obtained from the pregnant patient in the late first or early second trimester. Electronic fetal monitoring may show an initial tachycardia, rapidly followed by decelerations, bradycardia, and a preterminal sinusoidal rhythm. Chest radiographs were interpreted as "possible" pneumonia in 20% and as "unequivocal" pneumonia in only 4%. Many clinicians transfuse 5 to 15 mL of packed, washed, and irradiated maternal platelets (obtained by plateletpheresis) at the time of cordocentesis. The appropriate dose of valacyclovir for treatment of infection is 1000 mg twice daily. Postnatal surgery, which results in a far from optimal singleventricle Fontan-type circulation,314 has a considerable mortality rate, leading to a total survival of less than 65%. Intrapartum management of the parturient with a multiple gestation requires multidisciplinary cooperation. This local coagulation reaction is relatively protected from the dampening effects of circulating endogenous anticoagulants because of its intensity and because it is shielded by the initial layer of adherent and activated platelets. Patients should be instructed to refer their sex partners for evaluation and treatment. Early use of a bone marrow examination is helpful in pointing the investigation in the correct direction. However, if progress is slow or maternal or fetal status deteriorates, cesarean section should be performed. This index had a 94% positive predictive value for cesarean delivery of patients with abnormal labor patterns. The cervical length is measured, because it is a strong predictor of preterm delivery (with a cervical length of <20 mm, 60% are delivered before 24 weeks, and only 20% after 32 weeks). This saturated phosphatidylcholine is the principal surface-active component of surfactant. Pre-Conceptional Care for Women with Risk Pre-conceptional interventions to reduce risk for preterm birth typically target women with a previous preterm birth. Aghajafari F, Murphy K, Matthews S, et al: Repeated doses of antenatal corticosteroids in animals: a systematic review, Am J Obstet Gynecol 186:843, 2002. The use of cannulas obviates this problem, because the port remains during the entire procedure and different instruments and scopes can be introduced. Systematic reviews of the studies of most of these methods are included in the Cochrane Database. Anti-M and Anti-N Anti-M and anti-N are naturally occurring IgM antibodies that typically are cold agglutinins. This can be achieved by correcting the malformation, by arresting the progression of the disease, or by treating some of the immediately life-threatening effects of the condition, delaying more definitive repair until after birth. Furthermore, checklists are designed to improve team communication by presenting specific topics for dialogue relating to patient care. Traditionally, aortic valve replacement is not recommended until symptoms of heart failure (most notably exertional dyspnea) occur or left ventricular dysfunction or enlargement is seen on echocardiography. Tightening of the fundus with each contraction is detected by pressure on a small button in the center of the transducer, and uterine activity is displayed on the recorder. Pregnancy in patients with mild or moderate mitral regurgitation can be managed safely with a conservative regimen of reduced physical activity, salt restriction, and low dosages of a diuretic agent. For example, in a 50-kg adult human, the output of each ventricle would have to be 11. In one series, hypertension with hyperuricemia was as commonly associated with fetal growth restriction, as was hypertension with proteinuria. It is also possible to detect a fetomaternal hemorrhage by an abnormally elevated maternal serum -fetoprotein level.
