Impaired renal function and significant donor hypertension and diabetes are, for example, usually exclusion criteria based on donor safety rather than recipient outcomes. The most common causes include sepsis, neurogenic shock, adrenal insufficiency, and liver failure. Thrombotic thrombocytopenic purpura may also be induced by drugs, including ticlopidine, clopidogrel, mitomycin C, cyclosporine, tacrolimus, gemcitabine, and quinine, and the evidence for benefit of plasma exchange in this context is poor. Very high levels are seen in extensive acute hepatic necrosis such as in severe viral hepatitis and acute cholestasis.
