The iron chelator, deferasirox, has not been evaluated in heart failure patients and may be ill-advised in patients with marginal renal perfusion. The dose should be increased to the maximum tolerated, limited by hypotension in patients with thalassaemia. There is evidence that low levels of the enzyme epoxide hydrolase in amniocytes and fetal fibroblasts were associated with the fetal hydantoin syndrome. Clinicians participating in telemedicine need to be attuned to care continuity and coordination issues and to effectively convey to their patients who has accountability over their care and whom they should contact for follow-up. The occurrence of vomiting and abdominal pain should raise the suspicion of this complication.
