Diffusion-Weighted Imaging Diffusion of water in the brain depends on the integrity of the cellular membrane and the osmotic balance of the neurons and astrocytes. Dynamicmonitoring of brain activity can help intensivists limit secondary injury in brain-injured patients and has the potential to detect neurological injury in at-risk patients at the moment that it occurs. Many free radical scavengers and neuroprotective agents that have shown promise in the laboratory or in animal models have generally failed in human trials. There are some reports of localized hyperperfusion in ictal studies of patients with frontal, parietal, or occipital epilepsy. About 2% will have clinical overdrainage despite a valve pressure setting of 200 mm H2O, and the other 2% will have underdrainage at a setting of 30 mm H2O. In patients taking ethambutol, optic neuropathy occasionally develops, so those receiving long-term ethambutol treatment should be screened at intervals for visual acuity and color discrimination. Although mesial temporal lobe epilepsy has become better defined as a clinicopathologic entity that can be treated with a standard surgical approach,2,3 such is not the case for extratemporal lobe epilepsy syndromes. It should be exposed by aspiration of the fornix, dentate gyrus, and uncal tissue so that the entire width is visualized. There is increasing evidence that the perisylvian hemispherotomy method and associated disconnection techniques (transsylvian keyhole and vertical parasagittal hemispherotomies) are associated with an even lower rate of complications, shorter operative time, and less blood loss than the older methods. In contrast, ablative surgery does not require a second operation for battery implantation, and device-related complications, such as infection or adverse stimulation effects, are not a factor. First, unobstructed anteroposterior and lateral radiographs or fluoroscopy can be obtained. The programming wand in a sterile drape is then introduced into the operative field and used for diagnostics and programming.
