Extrahepatic disease is not a contraindication to liver-directed therapy when the liver tumor burden dominates the clinical outcome. The cytotoxic T cell then releases perforin, which kills the attached target cell by making it leaky. These techniques measure the global perfusion of the extremity by recording the minute changes in the volume of the extremity that occur throughout the cardiac cycle. Because of this increased filling, the FrankStarling mechanism also contributes to the increased stroke volume, although not to the same degree as the increased contractility. The choice of injection levels is based on the location of focal tenderness over the joints or the presence of osteoarthritis involving the joints. Chapter 15 describes how bile salts are normally absorbed from the small intestine so that very few of those secreted into the bile are normally lost from the body. As we will see, the most common causes of secondary amenorrhea are pregnancy and menopause. As many as one third of patients also have focal, discrete aneurysms superimposed on generalized ectasia, especially of the femoral and popliteal arteries. Typically, connectivetissue involvement is extensive, with repeated episodes of inflammation in joints and skin. Although, theoretically, hypertension could result from an increase either in cardiac output or in total peripheral resistance, it appears that in most cases of well-established primary hypertension, increased total peripheral resistance caused by reduced arteriolar radius is the most significant factor. The usual diseases in industrialized nations leading to cirrhosis and portal hypertension are alcoholic liver disease, chronic viral hepatitis (hepatitis B and C viruses), and steatohepatitis (Box 14-2).

Excessive amounts of those chemicals caused widespread injury to the microvascular endothelium and led to leakage of fluid out of capillaries. Depending on the particular substance, reabsorption and secretion can occur at various sites along the tubule. When this is not possible, use of a long, flexible sheath or guiding catheter placed over the aortic bifurcation is necessary to provide stability and allow contrast injections. This explains, therefore, why the partial pressure of oxygen in air at sea level is equal to 0. Patients complain of severe pain persisting after deflation of the balloon, which may be accompanied by hypotension and tachycardia.
