The left phrenic nerve enters the thorax medial to the vagus nerve and continues downward on the pericardium. When the continuity of the aorta has been reestablished, further perfusion and rewarming can be achieved through the femoral or preferably the aortic cannula. Static compliance is the compliance measured when the infant is completely passive and can be estimated using an inspiratory hold at end inhalation during assisted ventilation (McCann et al, 1987). In infants and children, right atrial pressure is estimated to be between 3 and 10 mm Hg, with 5 mm Hg typically chosen as an estimated guess. It is also associated with persistent chest wall deformity, which might be due to the severity of the underlying disease or the complication of reherniation (requiring multiple surgical procedures). Lung biopsy is probably not indicated and can be hazardous because once the distended lymphatic channels are severed, they can leak fluid for weeks. Conditions of measurement Conditions under which self-measurement is performed can affect the measured blood pressure levels. The ductal opening is then oversewn under direct vision in a relatively dry field. Generally, medialization of the vocal cord is not recommended in neonates under 6 months of age, with tracheostomy preferred in severe cases of respiratory obstruction (Parikh, 2004). Fibrous adhesions to the undersurface of the sternum are mainly along the previous sternotomy. Herpes episodes are described as primary (with no previous exposure at any site), first episode/postprimary (following previous covert exposure), or recurrent. The axillary artery is usually soft and the preferred site for arterial cannulation (see Chapter 2 for axillary artery cannulation). Significant longterm complications reported include: vesicovaginal or rectovaginal fistulae; rectal stricture; and vaginal stenosis.
