A few thickened interlobular septa are present, which makes differentiation from pulmonary veno-occlusive disease challenging. A new strategy for the assessment of viable myocardium and regional myocardial blood flow using 15O-water and dynamic positron emission tomography. On a practical level, redistribution is manifested as a "filling in" of a myocardial perfusion defect occurring 3 to 5 hours after the injection of 201Tl at peak stress. Modern Doppler devices use pulsed Doppler systems and have color Doppler, spectral Doppler, and power Doppler options. When additional scanning parameters such as contrast injection rates are taken into consideration, the results are often spectacular, with minimal contamination from nonportal vascular opacification. The left ventricle is severely volume overloaded, leading to congestive heart failure. Analysis of ventricular contraction by factorial phase imaging with equilibrium radionuclide angiography. Generally, vertical motion errors are detected and corrected more reliably than horizontal and bulk body motion errors. If these two events happen to occur at the same time, the event is termed a multiple. Oscillating intracardiac mass on valve or supporting structures, in the path of regurgitant jets, or on implanted material in the absence of an alternative anatomic explanation, or b. Key determinants of long-term outcome in mitral stenosis include the degree of pulmonary hypertension, the presence of atrial fibrillation, and left ventricular function. If a large shunt is present and persists longer than 6 to 8 weeks, pulmonary artery banding and removal after age 2 years with an attempt at septation is reasonable. The injury involves a tear of one or more layers of the aortic wall and is usually located at the distal aortic arch or proximal descending thoracic aorta. Because the number of blood cells per unit blood volume is more or less constant, it is the spot volume of the ultrasound beam that determines the strength of the Doppler signal. Corticosteroids, hydroxyurea, cytotoxic drugs, and imatinib all have been employed, with variable results. B, Immediately after extraction thrombectomy, prompt restitution of flow is noted in the large vascular bed supplied by this bypass graft. In general, images in infants are of higher quality than those in older children and adults. Hepatic Veins the hepatic veins drain the liver parenchyma and do not contain valves. Although this time window is influenced by the injection/infusion protocol used, the dosage, and cardiac output, it is most importantly dependent on the kind of contrast agent used-a first pass agent or a blood-pool agent. However, some patients with type B aortic dissection may require surgery when the patients have hemodynamic instability, intractable pain, rapid expansion of aortic diameter, and mediastinal or periaortic hematoma as signs of imminent rupture of the aorta. To optimize image quality, it is necessary to understand why artifacts occur and how they can be prevented (Table 35-1). In one series of four patients with contrast agent extravasation from thoracic aortic trauma, none survived. Branch pulmonary artery stenosis decreases perfusion to the affected lung and, if severe, causes hypertension in the nonaffected lung and right ventricle. If a severe perfusion defect on initial rest imaging does not improve on the redistribution study ("fixed"), this correlates well with infarction. The severity of symptoms does not always correlate with the degree of stenosis, and some patients with moderate stenosis become symptomatic, whereas others with severe stenosis by imaging criteria remain without symptoms. B, Gadolinium-enhanced axial black blood image shows only mild enhancement of the septal wall (arrow), consistent with the diagnosis of hypertrophic cardiomyopathy. In the presence of other vessel running close to the vessel segment of interest or stenotic areas where the maximum intensity at the location of the pathline is low, overestimation of the lumen contour may occur in individual slices.
Echocardiography Echocardiography is a simple and useful modality in the evaluation of pericardial effusion, hematoma, cardiac tamponade, and intracardiac thrombus. Histologic confirmation of the diagnosis after right ventricular biopsy entails visualizing endomyocardial fibrosis, thrombus, granulation tissue, or chronic inflammation from eosinophilic infiltration. Currently, 85% of children with tetralogy of Fallot survive to adulthood, increasing the prevalence of tetralogy of Fallot in adults. Anterior pulmonic cusp Left pulmonic cusp Left aortic cusp Anterior mitral leaflet Posterior mitral leaflet Right aortic cusp Posterior or non-coronary aortic cusp Anterior tricuspid leaflet Septal tricuspid leaflet Posterior tricuspid leaflet Postoperative Surveillance Routine (scheduled) postoperative radiographic surveillance is typically not performed unless the patient has new findings consistent with cardiac ischemia, such as angina, shortness of breath symptoms, a positive exercise stress test, or a change in myocardial contractility on echocardiogram. The aortic root position is typically anterior and to the left of the pulmonary root. In two clinical trials that directly compared dobutamine stress myocardial perfusion imaging with stress echocardiography, the sensitivities were 86% and 80%, and specificities were 73% and 86%. Ultrasonography also evaluates invasive arteriographic complications such as pseudoaneurysms, hematomas, and arteriovenous fistulas. The perioperative mortality rate is approximately 10% and is half that of an open repair. Because of a lower prevalence of disease, they recommend against screening of women (or screening only women with multiple cardiovascular risk factors) or younger or nonsmoking men. Pericardiotomy or pericardiectomy may be indicated in patients with metastatic pericardial disease. The ratio of the ankle to the higher of the brachial systolic blood pressures is obtained; a ratio below 0. In normal embryologic development, beginning in the fifth week of gestation, fusion of the endocardial cushions separates the atrioventricular canal into right and left openings, which are destined to become the tricuspid annulus and the mitral annulus. Myxomas are pedunculated mobile masses on echocardiography, often apparently attached to the interatrial septum by a narrow stalk. To simplify the selection, the user needs to define a proximal (start) and a distal (end) point inside the vessel of interest in the 3D data space. Ultrasonography Abnormalities on echocardiography, including increased or decreased wall thickness, ventricular dilation, functional impairment, mitral regurgitation, impaired diastolic relaxation, and pericardial effusion, have been reported in 14% to 40% of patients with sarcoidosis. Angioplasty has an 80% technical success rate, with an 80% rate of clinical remission at 2 to 3 years. Long-term patency rates are improved if the underlying lesions are treated promptly by surgery or percutaneous techniques. Depending on the location of the stenosis or occlusion, an antegrade or retrograde approach is undertaken. Contraindications Adult patients after repair of complex congenital heart disease such as tetralogy of Fallot or truncus arteriosus are complex, often with multiple anatomic, hemodynamic, and arrhythmia issues in addition to their branch pulmonary artery stenosis. The recurrence rate is approximately 25%, and recurrence is frequently associated with complications such as aortic rupture and death from exsanguinaton. On imaging studies in these individuals, the internal iliac artery is larger than the external iliac artery. The first-pass extraction fraction of ammonia N 13 is very high at approximately 95%. Right ventricular enlargement on chest computed tomography: prognostic role in acute pulmonary embolism. This increase is primarily attributed to more sensitive detection through echocardiography. These symptoms are relatively mild and generally selflimiting; support with oral or, rarely, intravenous hydration and oral analgesics is usually sufficient. B, Following septal ablation, a significant perfusion deficit is noted in the septal wall (arrowheads), depending on patient chosen). Because of a small incidence of atrial arrhythmias after device placement, a baseline electrocardiogram should also be obtained. The low perfusion of cardiac tracers in the extremities makes these artifacts difficult to visualize. Contrast-induced renal failure occurs less than 3% of the time in patients without significant risk factors, 5% to 10% of the time in patients with diabetes or renal dysfunction, and in 10% to 50% of those with both comorbid conditions.
Contrast agents for magnetic resonance angiography: current status and future perspectives. Furthermore, patients with preexisting primary conditions such as pregnancy and renal failure are contraindicated from the use of this test. Many different brands are available, all of which are generally equal in efficacy. First, the patient should be rescanned as soon as possible after the repeat bleed comes to clinical attention. When it is eccentric, along the vessel wall, acute pulmonary embolism is convex toward the lumen. Coexistence of subtricuspid and anterior infundibular wall bulging and hypertrophic trabeculae has been shown to have 96% sensitivity and 87. Mesenteric arteriography is also performed as part of an endoleak repair if the inferior mesenteric artery is the source of the endoleak. The iliolumbar artery extends laterally and superiorly to divide into two branches that supply the iliacus muscle and ilium (iliac branch) and the psoas major and quadratus lumborum muscles (lumbar branch). If the vessel lumen cannot be visualized because of shadowing from plaque, an approach to the vessel from different angles (including the transverse plane) or use of a curved array transducer may be helpful. The positron will scatter around in the body (within a millimeter or so of where the decay took place) until it meets an electron, and then they are annihilated. The bisferiens waveform may not be visualized in all vessels and may be visualized only intermittently in some patients. Outback, Cordis), an ultrasound-guided re-entry needle for subintimal angioplasty, thermal ablation devices such as a radiofrequency ablation wire. Flow analysis and visualization for cine three-directional volumetric velocity mapping has gained significant interest but is currently limited to research applications, partly because of the lack of intuitive analysis platforms. Circulation 1992; 86:167-178; with permission from the American Heart Association. Along its course in the forearm, the cephalic vein receives dorsal and volar tributaries. Marfan syndrome is the most common of the connective tissue processes, with an incidence of 1:10,000. The superficial vein drains the prepuce and skin of the penis into the external pudendal vein. Immunosuppressants such as prednisone, cyclosporine, and azathioprine may be helpful in selected patients, particularly those with myocarditis related to autoimmune disease or giant cell myocarditis. It gives rise to multiple small muscular branches and, just superior to its entrance into the adductor canal, it gives rise to the descending (supreme) genicular artery. Cryoplasty for femoropopliteal arterial disease: late angiographic results of initial human experience. Ebstein Anomaly Downward displacement of tricuspid valve into the right ventricle Caused by anomalous attachment of tricuspid leaflets Valve tissue is dysplastic and results in tricuspid regurgitation. Several projections obtained using fluoroscopic guidance allow the surgeon to obtain a three-dimensional assessment of the coronary arteries (Table 30-3). The Society for Vascular Surgery24 and the American Heart Association25 have therefore recommended optimal medical management for all patients with carotid stenosis. Suggestive clinical context I Neurogenic pulmonary edema (intracranial pathology) I Re-expansion edema (rapid drainage of pneumothorax or pleural effusion) I High-altitude pulmonary edema I Reperfusion edema following lung transplantation I Edema associated with tocolytic therapy (preterm labor treatment) I Postpneumonectomy or postvolume reduction edema, air embolism. Image postprocessing requires modification of three-dimensional data to derive additional information or to hide unwanted information. Discussion with the patient on what to expect and the need for the patient to communicate any unusual sensations immediately is imperative. This technique is achieved by interrogating the same sample volume multiple times (at least 3, typically 10 to 20 times), and correlating the echoes. Percutaneous transluminal septal myocardial ablation for hypertrophic obstructive cardiomyopathy: long-term follow-up of the first series of 25 patients. Diagnostic accuracy of rubidium-82 myocardial perfusion imaging with hybrid positron emission tomography/computed tomography in the detection of coronary artery disease. Using these criteria, the average positive predictive accuracy for predicting improved segmental function after revascularization is 76% (range 52% to 100%), whereas the average negative predictive accuracy is 82% (range 67% to 100%). In older adults, it is used only in cases of palliation in poor surgical candidates or as a temporary bridge to valve replacement in clinically unstable patients. The greater these assumptions are violated, the greater will be the negative impact on the final image.
Intravascular stents in congenital heart disease: short- and long-term results from a large single-center experience. However, the pathline found using this approach will not be at the desired center of the lumen at every location. Sagittal oblique plane in the long axis of the aorta should be obtained, the so-called candy cane view. Through these two openings, blood flow develops into two intertwined, spiraling streams that flow out the conus cordis and the truncus arteriosus, or conotruncus for short. On occasion, patients are so asymptomatic that the diagnosis is not made until they have reached adolescence or adulthood. Radiation dosimetry of nuclear radiopharmaceuticals is calculated by a mathematical biokinetic model that incorporates the tissue biodistribution of the agent as well as biologic and physical decay. A study by Haraikawa and colleagues46 included 20 patients with various kinds of heart disease. Image fusion can be used to track catheter movement and to obtain precise anatomic localization of ablation points. However, the repeated cycles of inflammation, smooth muscle cell and fibrous tissue proliferation, and expansion of the lipid core eventually overwhelm the compensatory response, leading to progressive luminal obstruction. Surgical intervention ranges from open biopsy to palliative debulking to wide surgical resection. Additional cardiac findings include heart block owing to injury of the conduction system, often associated with aortic root abscess and coronary ischemia secondary to emboli. Long, smooth, tapering stenoses with areas of dilation may also be seen in subclavian, axillary, and brachial arteries. The three adrenal arteries are refined during the process of kidney ascension, which occurs by the sixth week of development. Although they are rare presentations, surgical intervention may be delayed for a few days to allow further diagnostic evaluation, if indicated, or when other medical considerations exist. These factors favor the production of hydrostatic pulmonary edema when the pulmonary collapse is relieved. B, Caudal to A, three vessels are seen in the following order, from left to right: left innominate (red arrow), right common carotid (asterisk), and right subclavian artery (yellow arrow). Coronary hyperemia induced by higher doses of dobutamine depends on increasing myocardial oxygen demand in response to increases in cardiac contractility and heart rate. For thoracoabdominal aortic aneurysms due to atherosclerosis, the rupture rate is 18% at 2 years for aneurysms greater than 5 cm. This is accomplished by extracting patterns from the image information as well as by corresponding output classes as assigned by the expert segmentation. Stress imaging begins approximately 15 minutes to 1 hour after the stress injection of 1. On any slice in the phantom field of view, the activity profile across the diameter should be flat. A, Posteroanterior examination showing typical signs of shunt vascularity and cardiac enlargement. By summing all of the signals from all of the photomultiplier tubes in an array, it is possible to know the total amount of light for a given scintillation event. Following gadolinium (B), the endoleak is visible adjacent to one of the iliac limbs (arrow). Postoperative Surveillance Postoperative surveillance is similar to that for endovascular interventions and is discussed in a separate chapter. Patients, especially patients with small or isolated defects, are usually asymptomatic through the first 3 decades of life, although more than 70% become impaired by the fifth decade. A maximal stress test may satisfy diagnostic purposes if it goes beyond the hemodynamic threshold of triggering the ischemic symptoms. Even when penetration is successful, entry into the distal lumen can be problematic, and dissection of the distal vessel is common.
Diseases
Chronic effusive pericarditis is related to numerous diseases, including renal disease, hemodialysis, malignancy, medications, and (rarely) infection, and is more common in young women. Characteristic x-rays (top) and Auger electrons (bottom) can be emitted during electron transition. As blood flow and oxygen supply decline, oxidative metabolism decreases, and glycolysis increases. Angiography Catheter arteriogram of the aorta is the gold standard to confirm abdominal aortic injury. Although differences exist between these agents in terms of the molecular structure and chemical and physical properties (Tables 18-1 and 18-2), all agents are nonspecific and are eliminated unchanged via the renal pathway by glomerular filtration. There is relative sparing of its posterior margin consistent with an inflammatory abdominal aortic aneurysm. If cardiac motion is detected, additional phases should be reconstructed, trying to find a motion-free phase (if the technologist is not trained to perform this assessment and to look for the best phase for the reader). Intracardiac echocardiography using the AcuNav ultrasound catheter during percutaneous balloon mitral valvuloplasty. Prevalence and Epidemiology the reported incidence of cor triatriatum ranges between 0. Angiography Angiography remains the gold standard for pressure studies and pulmonary arterial and vascular resistance studies. Chronic thromboembolic pulmonary hypertension-assessment by magnetic resonance imaging. The internal pudendal artery courses inferiorly along the anterior surface of the piriformis muscle, lateral to the inferior gluteal artery, and enters the ischiorectal fossa through the lesser sciatic foramen. Osteosarcoma Definition Cardiac sarcomas with osteosarcomatous differentiation arise almost exclusively (>95%) in the left atrium. Because each pixel carries its own displacement information, there are numerous measurements made within the myocardium. Rather, invasive coronary angiography with the option of direct intervention and revascularization is the more appropriate initial procedure in the management of such patients. The reflected and scattered ultrasound waves are collected with transducer arrays and converted to an image, which reveals the anatomic structures of the body. Experience with in situ saphenous vein bypasses during 1981 to 1989: determinant factors of long-term patency. Activation of the adenosine A2 receptor leads to a complex series of events mediated by G proteins and leading to a decrease in intracellular calcium uptake, culminating in coronary vasodilation. In some cases, provocative maneuvers will deepen the midsystolic retraction in a pre-steal waveform, causing a transient reversal of midsystolic flow or complete flow reversal in the vertebral artery. The received signal is digitized by an analog to digital converter and a discrete inverse twodimensional Fourier transform generates the corresponding image, which contains magnitude (length of the transversal magnetization) and phase information (position in the transverse plain in respect to the coil axis) for each voxel. Additional factors, such as adequacy of collateralization, plaque ulceration or hemorrhage, hypotension, and low cardiac output, also play a contributory role. These veins conform to the same anatomy as the pancreaticoduodenal arteries, with anterior and posterior venous arcades between the superior and inferior pancreaticoduodenal veins. The highest signal can be obtained with a flip angle of = 90 degrees, which tips the magnetization vector completely into the transversal plane. It is important to understand the location, extent, and size of the diseased segment and characteristics of the plaque. The known relative limitations due to heavy calcifications are unlikely to be overcome soon because voxel size as a determinant factor of blooming artifacts is at the limits of the technically feasible spatial resolution at sustainable radiation dose. Several different closure devices are currently used because of the significant variability of ductal anatomy. Segmented k-space breath-hold cine imaging has an in-plane spatial resolution that typically ranges from 1. It is the associated congenital cardiac defects that often increase the potential morbidity and increase the need for a diverse approach to monitoring of these patients postoperatively.
Vacuoles containing red blood cells may be seen, and most lesions contain areas of necrosis. The true lumen is generally posterior and left posterolateral in the ascending aorta, caudal in the aortic arch, and anterior and right anterolateral in the descending aorta. A, 83-year-old woman with history of transient ischemic attack demonstrates a mild stenosis (grade 2, 10% to 50%) at the origins of the left internal carotid artery (black arrow) and significant stenosis (grade 3, 51% to 99%) at the origin of the left external carotid artery (white arrow) on 3. The scanning volume starts at the aortic arch and extends caudally to the apex of the heart. The variant renal and suprarenal blood supply with data on the inferior phrenic, ureteral and gonadal arteries. Because of design of the transducers, weak secondary lobes of focused sound may insonate areas unrelated to the primary beam, and if reflected echoes from these regions are strong enough, they are displayed in the image. Other types of collimators include slant-hole, converging, diverging, and pin-hole. The absence of change between studies before and after acetazolamide administration indicates an absence of cerebrovascular reserve. Maintenance of a compact contrast bolus with the effect of optimizing the T1 difference between enhanced blood and adjacent soft tissues. Lipid-lowering therapy is another essential component to retard the progression of atherosclerosis. Also as a result of minimal redistribution, images may be acquired at a time remote from injection, permitting the potential use in patients with acute coronary syndromes. Adequate preparation of the patient is a must, and therefore a team made up of scheduling staff, technologist, physician, and nurse is required. Postprocessed viewing of data provides a surgeon or interventionalist with a valuable three-dimensional perspective of the aneurysm, its location, and its position relative to its branch vessels and adjacent structures. This makes a careful physical examination and targeted noninvasive imaging even more important. Etiology and Pathophysiology the pathology of many cases of intramural hematoma is spontaneous rupture of the vasa vasorum, which results in bleeding into the aortic media. The great saphenous vein is present in both the leg and thigh, whereas the small saphenous vein most commonly only drains the calf. Clinical utility and safety of a protocol for noncardiac and cardiac magnetic resonance imaging of patients with permanent pacemakers and implantablecardioverter defibrillators at 1. Ongoing research and technical advancement may identify relevant metabolic tracers and increase the role of these imaging modalities in the future. B, this is demonstrated in a patient with Takayasu arteritis in whom the aortic wall is thickened (arrow) and displays increased signal intensity in T2-weighted black blood images resembling those in aortitis. As ischemia continues, glycolytic metabolism becomes overwhelmed with excess production of lactate. Note the failure to augment between the brachial and femoral pressures, signifying aortoiliac disease on the left, and blunting of the waveform below the femoral artery, signifying left superficial femoral artery disease. Imaging of the portal venous system can be achieved indirectly through arterial portography or directly through transhepatic portography, splenoportography, or catheterization and injection of the umbilical vein. In this case, partial anomalous pulmonary venous return is usually present and it affects the right upper pulmonary vein. Four-detector row computed tomographic angiography in the evaluation of infrarenal aorta and peripheral arterial occlusive disease: influence of contrast medium concentration. The modern concepts and techniques of extracorporeal circulation were refined further by Lillehei and Kirklin in the early 1950s. The choice between adenosine and dipyridamole is generally given to adenosine because of rapid onset of action and short halflife, lower cost per dose, and easier monitoring after study completion. The inferior mesenteric artery, if patent, may be ligated in the presence of strong backbleeding or reimplanted in the presence of poor backbleeding. Sometimes overhead is not possible, so strict instructions about stillness should be emphasized. Solid-state, semiconductor-based detectors, such as cadmium zinc telluride, have the potential to improve energy and spatial resolution, decrease scan times, reduce radiation exposure, and allow for simultaneous dual-isotope imaging in small footprint imaging systems.
Because of this, the fraction of recorded events that are scattered in a threedimensional scanner can approach 50%. There is a great morphologic heterogeneity of atherosclerotic plaque, which reflects the stage of progression and the variety of pathways involved after intimal injury. After establishing cardiopulmonary bypass, the pulmonary arteries from the common arterial trunk or the aorta are identified and excised. Intracoronary stenting for acute and threatened closure complicating percutaneous transluminal angioplasty. In pediatric patients weighing less than 10 kg, an empiric scan delay of 12 to 15 seconds after the start of the intravenous contrast injection suffices. Each readout is represented by a dot and the readout direction is perpendicular to the shown ky-kz plane. The first and second aortic arches contribute to the formation of the stapedial artery. Delayed recovery of fatty acid metabolism after transient myocardial ischemia: a potential imaging target for "ischemic memory. A and B, Short-axis end-diastolic (A) and end-systolic (B) frames reveal a dilated left ventricle with poor function. Concomitant low-dose heparin therapy is administered through the arterial sheath or an intravenous route. Other limiting factors in exercise stress testing are physical deconditioning, peripheral vascular disease, history of stroke, lower extremity amputation, and severe chronic obstructive pulmonary disease. Although Doppler developed his hypothesis for light waves, he also noted that the same hypothesis applied to sound waves. Nitinol stents have a thermal memory, which allows them to expand to a predetermined diameter at body temperature. Imaging Techniques and Findings the sensitivities and specificities of the multiple imaging modalities that are used to assess noncoronary atherosclerotic disease are presented in Table 88-3. However, if there is an aberrant right subclavian artery, the pressure in the right upper arm may be equal to or lower than that in the left upper arm. Developments in nuclear cardiology: transition from single photon emission computed tomography to positron emission tomography-computed tomography. Cardiovascular T2-star magnetic resonance for the early diagnosis of myocardial iron overload. These are most useful for quickly displaying the anatomy of the vascular abnormality and its relationship with adja- cent structures. A perfusion defect and delayed hyperenhancement are noted after treatment, clearly demonstrating the location and extent of infarcted myocardium. Transluminal treatment of arteriosclerotic obstruction: description of a new technique and preliminary report of its application. Note the decreased blood pool to myocardium contrast and apparent loss of resolution. Patients with infective (mycotic) pseudoaneurysms may be asymptomatic or may present with fever, lethargy, and chest pain. There is a larger and more developed dorsally positioned caudal tract (the right and left posterior cardinal veins) and a more ven- trally located cranial tract (the right and left anterior cardinal veins). Alternatively, quartile gradations can be used, such as 0% to 25%, 26% to 50%, 51% to 75%, and 76% to 100%. Validation of computed tomography image integration into the EnSite NavX mapping system to perform catheter ablation of atrial fibrillation. Therefore, it should not be a surprise that a drug that depends on renal elimination has the potential to change its biologic behavior if the pathway is impaired, consequently making agents with multiple or other elimination pathways highly desirable for patient populations with renal impairment. There are limited randomized trial data comparing revascularization to medical therapy. A pulmonary arteriolar resistance of more than 8 Wood units obtained during cardiac catheterization with pulmonary vasodilation is also a contraindication to surgery. Ron Fairman, Chief, Division of Vascular Surgery and Endovascular Therapy, Hospital of the University of Pennsylvania.
In approximately 3% of limbs, the anterior tibial artery arises from the popliteal artery superior to the popliteus muscle (high origin). Generally speaking, all patients who are symptomatic and do not have a contraindication should be treated surgically. In this mode, the responding detectors and time of every event are stored in a very large data file. Because of the time required to do manual planimetry, automatic and semiautomatic methods have been developed. Visualization of the direction of flow characterizes the type of endoleak definitively. The goal of imaging to detect thin cap fibroatheroma and distinguish it from fibroatheroma with thicker caps remains elusive, but may eventually enable the prophylactic treatment of lesions prone to rupture with the aim of preventing atherothrombosis. Atrial myxomas, if calcified, may be occasionally seen within the left atrium on chest radiographs. Contralateral occlusion was the only predictor for moderately increased perioperative risk of stroke and reduced long-term survival. Orthotopic cardiac transplantation has emerged as the most reliable long-term treatment option for patients with endstage heart failure, despite maximum revascularization and medical therapies. Furthermore, an adaptive dose shield can block irrelevant pre-spiral and post-spiral radiation with dynamic diaphragms, thus ensuring that only a minimum and clinically essential radiation exposure occurs. A, B, Frontal and lateral chest radiographs (arrows) demonstrate a pulmonary valve ring prosthesis. Extravasation should be avoided because this can result in soft tissue swelling and tissue necrosis. Predicting the risk of early atherosclerotic disease development in children after repair of aortic coarctation. Right Ventricular Function Magnetic Resonance Imaging Normal Values Tandri and associates36 examined 500 subjects free of cardiovascular disease who were participants in the MultiEthnic Study of Atherosclerosis. This technique has also been shown to be useful in diabetic patients, in whom the ankle-brachial index is unreliable. Anomalous origin of the left coronary artery from the pulmonary trunk: its clinical spectrum, pathology and pathophysiology, based on a review of 140 cases with seven further cases. Although the incidence of progression of thin cap atheroma is unknown, at least 6% of patients undergoing percutaneous coronary intervention develop progression of disease requiring intervention for a different target lesion within 1 year. Surgical/Interventional Patients with hemodynamic instability or uncontrollable pain should be recommended for surgical treatment. Coronary Flow Reserve As atherosclerosis develops in epicardial coronary arteries, the resulting stenosis produces resistance to blood flow. Aortic dissection can also be caused iatrogenically by aortic surgery or percutaneous procedures such as catheterization and placement of intra-aortic balloon pumps. For patients undergoing elective repair of ascending aneurysm, this study provides accurate information regarding the size, location, and relationship to the branch vessels of the aorta and can depict the presence of aortic valve regurgita- Outcomes and Complications Operative mortality for elective repair of thoracic aortic aneurysm is very low, in most series 2% to 4%. Mesenteric Atherosclerotic Disease Because of its vague symptoms and infrequent diagnosis, there are no good studies outlining the true prevalence or natural history of mesenteric arterial disease. He is the founder of the Institute for Magnetic Resonance Safety, Education, and Research and is a fellow of the International Society for Magnetic Resonance in Medicine. The left subclavian (1 arrow) and left bracheocephalic (2 arrow) veins and superior vena cava (3 arrow) have been opacified. Relative contraindications include renal impairment, hepatic impairment, recent transient ischemic attack, diabetic retinopathy, pregnancy, thrombocytopenia, and coagulopathy. Regional wall motion is graded by visual assessment because quantification has not been shown to be superior to visual assessment. Sarcoidosis is unique among potential life-threatening disease because many patients do not require treatment. In patients older than 70 years, the incidence occurs with equal frequency in men and women. Noninvasive studies typically yield information about the lesion that is reliable enough to plan further management. The initial imaging examination is usually chest radiography and echocardiography. Radiation absorbed dose estimates to the embryo from some nuclear medicine procedures. Murmurs in both conditions are related to increased blood flow across the pulmonary and tricuspid valves.
